Prosthodontics Patient Registration In Duluth MN

How to Become a Patient at Dental Implant & Reconstructive Center

If you are interested in becoming a patient at Dental Implant & Reconstructive Center, the first step is to contact our office or request an Initial Consultation. A referral from another dentist is not required.

When you choose to schedule, a member of our team will speak with you by phone and collect the information needed to register you as a patient, including your name, date of birth, and a mobile phone number or email address.

Not ready to schedule? That’s okay. You are welcome to learn about our practice and treatment options before deciding whether you would like to schedule an Initial Consultation.

Already have Existing Dental Implants?

Please tell our team when scheduling. We will provide an electronic or paper Release of Information (ROI) form so we can request records from your previous providers.

Of particular importance are your implant “stickers”—the identifying labels taken from the medical-device packaging when your implants were placed. These should have been saved or scanned into your dental chart at the office where your implants were placed. We also request available information about your abutment system and restoration design.

These records help Dr. Esguerra identify the components involved and plan appropriate care.

Can’t remember the doctor’s name or where your implants were placed?

No worries. Original implant “stickers” give us exact identifying information, but we also have a specialized protocol for identifying existing implants and compatible components using X-rays. Our estimated identification accuracy with this approach is approximately 90%, so we can often help even when the original records are unavailable.
