Preparing for Your Initial Consultation

Dental Implant & Reconstructive Center in Duluth MN

WELCOME TO OUR PRACTICE

We want to thank you for visiting the Dental Implant & Reconstructive Center website! Our goal is for your first visit to be one of comfort and acknowledgement of your personal dental needs.

Whether you were referred by your dentist or found our practice on your own, we want you to feel comfortable contacting us. A referral is not required to schedule an Initial Consultation.

Our goal is to understand what brings you to our office, what matters most to you, and how we can help. From your first phone call through your consultation, we want you to know what to expect and feel prepared for each step.

Getting Scheduled

When you call, a member of our team will ask a few questions about your dental concerns and the reason you are seeking care. We will explain the Initial Consultation, gather the information needed to register you as a patient, and help you schedule your appointment.

Once your appointment is scheduled, we will mail you a welcome letter, information about our practice, and detailed parking instructions. You will also receive two electronic forms to complete before your visit: your Health History and Office/Financial Policies.

Initial consultation fee

The fee for your Initial Consultation is $325 plus applicable Minnesota tax. Your consultation is a comprehensive, approximately 90-minute visit designed to help us understand your condition, concerns, and treatment goals. For most patients, a 3D CBCT scan and specialist radiology review are included at no additional charge.

A $50 scheduling deposit reserves your appointment and is applied toward your consultation fee. The deposit is refundable when an appointment is canceled or rescheduled more than 48 hours in advance.

PREPARING FOR YOUR VISIT

Please complete your electronic forms before your appointment. We reserve approximately 90 minutes for your Initial Consultation, and completing your forms in advance allows us to use that time to focus on you, your concerns, and your treatment goals.

If you have difficulty completing the forms electronically, please arrive 15 minutes early. An iPad is available in our office, and our team can assist you.

What to Bring

Please bring:

  • A current medication list, including dosages whenever possible.
  • Any dentures, partial dentures, nightguards, or other dental appliances you have used in the past—even if you no longer wear them. Examining previous appliances can provide valuable information about what worked well, what did not, and why.
  • A photo ID.
  • Your dental insurance card, if applicable.

Consider Brining a Support Person

We welcome and encourage you to bring a spouse, family member, friend, or other support person to your Initial Consultation.

Whether that person provides emotional or financial support—or simply serves as a second set of ears—having someone you trust with you can be helpful. There can be a lot of information to take in, and we want you to feel comfortable asking questions, discussing your goals, and considering your options.

PREVIOUS DENTAL RECORDS

If you were referred by another dental provider, they may send records or X-rays to our office before your consultation. Dental records are not automatically shared between dental offices, so we may not have records from your previous providers unless they have been specifically sent to us.

You do not need to obtain previous dental records before your Initial Consultation unless our team asks you to do so.

Existing Dental Implants

If you already have dental implants, information about your previous treatment can be important to your ongoing care. There are many different implant systems and components, and they are not universally interchangeable.

We may ask you to complete a Release of Information so we can request records from the provider who placed your implants. If possible, please provide the name and location of the dentist or specialist who placed them.

Not having these records does not prevent you from scheduling a consultation, but additional information may be needed before certain maintenance, repairs, or treatment can be performed.

PARKING & ARRIVAL

Our office is located in the Medical Arts Building, 324 W. Superior St., Suite 1212, Duluth, MN 55802.

Several parking options are available, including metered street parking and the Medical Arts Garage. We validate parking ONLY at the Fourth Avenue Auto Park, located at 402 W. Michigan St. We do not validate parking in the Medical Arts Garage.

Detailed parking and arrival instructions are mailed to you after your Initial Consultation is scheduled. If you have difficulty finding our office, please call us at 218-722-8118.

We look forward to meeting you, learning what matters most to you, and helping you understand the options available for your care.
